We are dedicated to enhancing the lives of those we serve. That’s why we offer our residents and outpatients comprehensive therapy services to help promote a fast recovery and a better quality of life. If you or someone you care about is in need of physical, occupational, or speech therapy, our programs can help you in your recovery process.

Physical Therapy

Our experienced team of physical therapy professionals includes licensed physical therapists and licensed therapy assistants. Our therapy team makes it their mission to provide residents and outpatients with state-of-the-art therapy regimens while providing encouragement.

Occupational Therapy

With occupational therapy sessions, we focus on helping residents and patients get to a point where they can accomplish their personal goals, reach milestones, and ultimately, be able to function independently. Our program benefits individuals with physical, developmental, or cognitive and perceptual disabilities.

SpeECH therapy

Our speech therapists specialize in assisting residents and outpatients with a wide range of speech-related disorders and disabilities. Speech therapy services are available for individuals who are experiencing speech and language disorders, voice disorders, cognitive communication disorders, dysphagia, and other speech-related issues.

What can Skilled Rehab do for you?

Our community features a skilled rehab team who works with each patient and their family throughout the care process. We customize rehab plans so that the care reflects the individual’s needs and preferences. Our staff will assist with the transition through levels of recovery care.

What's the benefit to patients?

Skilled rehab can benefit patients by providing care for a number of situations including:

  • Rehabilitation care due to fractures or joint replacements
  • General wound care
  • Acute medical conditions
  • Interim medical care following a hospital stay
  • Respiratory care
  • Treatment for Parkinson’s disease
  • Stroke recovery
  • General rehabilitation due to weakness or physical decline
  • Diabetic rehabilitation
 

What will I do at a Skilled Nursing Community?

When you arrive, you will be assisted to your room and will participate in the admission process, which includes discussing your health and status. We understand that for the first day or so, everything is new for you so we encourage you to ask lots of questions and/or voice any concerns that you may have.

You will also be seen by a therapist on the day you are admitted. After your evaluation is completed, the therapy team will begin your individualized plan of care. This plan determines the frequency and duration of your therapy. We offer therapy services seven days a week along with group programs and individual activities to suit your interests. We design activities that encourage you to participate as much as possible in daily life during your stay with us.

What should I bring?

Bring comfortable, loose-fitting clothing. We recommend sweat pants and sweatshirts or shorts and t-shirts for therapy appointments. Please bring rubber-soled shoes as well. You’ll also need every day items such as undergarments, pajamas, socks, a robe, and slippers. You may also wish to bring a light sweater or jacket and your preferred toiletry articles.

How is Skilled Nursing different from a hospital?

Skilled nursing provides mostly short-term nursing or rehabilitation services. The typical patient has just been discharged from the hospital and still needs additional daily medical care prior to returning home. You might be recovering from a planned surgery or suffering from an unexpected medical crisis such as a hip fracture or serious infection. A hospital is a health care center where people are admitted because of accidents or various diseases, but stay there on a limited time basis.

long-term care​

The Long-Term Care program at Mission Health Communities serves those needing 24-hour nursing care for an extended period of time, while also providing a range of physical, speech, and occupational therapies as appropriate. We also provide respite, palliative, and hospice care.

resident enrichment

Whether tailored for individuals or designed with a group in mind, daily activities are an important aspect of life at our community. We encourage residents to improve their overall health and make new friends by participating in a variety of activites. Options include stimulating and therapeutic exercise classes and off-site outings to nearby restaurants or other attractions. We also host movie nights, sing-a-longs, and ice cream socials on a regular basis.

Dining & Meal Services

Nicely appointed dining rooms offer residents a fun way to socialize, meet new friends, and interact with others while they enjoy chef-inspired fare that can be tailored to meet dietary needs. Our dining staff is dedicated to making dining and meal services healthy and uplifting.

Worship

We offer ample opportunities for religious and spiritual enrichment including ecumenical worship services and access to spiritual care providers.

memory care

If cognitive impairment is becoming a concern, Mission Health’s Memory Care Program offers a safe residential solution. Memory care is a distinct form of care that caters specifically to patients with Alzheimer’s disease, dementia, and other types of memory problems. At our community, we offer secure, comfortable, and easy-to-navigate living areas. Daily care and activities are provided by staff who are specially trained to assist people with dementia or impaired cognition.

Our Memory Care teams are dedicated to stimulating the memories and senses of our residents through meaningful interactions and activities including:

• Creating an atmosphere of freedom, by offering an environment that is safe and encourages mobility
• Offering music-led therapy programming
• Providing social, spiritual, and recreational memory-focused programs to encourage interaction
• Encouraging communication with family members on an ongoing basis by providing support groups and programs that promote family involvement
• Offering on-site physical, occupational, speech, and respiratory therapy services

To determine and design the most personalized and effective program possible, we conduct a formal, pre-admission assessment for each individual to determine the appropriate level of care and services they’ll need.

We’re committed to providing you accessible information to make informed decisions about your health care and about our services. As part of that commitment, we use a national, standardized survey to measure our patients’ experiences. This survey is administered by the National Research Corporation (NRC). A brief survey is given to short-stay patients just prior to their discharge from our community. All of our other residents, patients and families are surveyed annually.

We use the results of these surveys to help improve our services for you. We also share this information with you to help you make a more informed decision. These patient surveys offer our patients an opportunity to rate their experiences and share comments, which are then displayed anonymously on this page.

We do not post comments that are libelous, slanderous, profane, or otherwise inappropriate or may risk the privacy of our patients or residents.
